Initial VisitPlease arrive approximately 15 minutes prior to your scheduled time. You will be asked to complete a brief patient information sheet. If you have insurance, you will need to bring your ID card for photocopying. Additional information including prescription medications, name and address of previous physicians and any pertinent records will help us give you the best care possible. Insurance PlansFamily Medical Group accepts most PPO insurance. We are also Medicare providers. The list below represents the majority of plans we are contracted with. If your health plan is not listed below, please contact your insurance plan to find out if our group is a contracted provider.
Calls to the OfficeIn order to expedite your weekday calls between 8 a.m. and 5 p.m., calls are received by our automated operator and will be transferred to the appropriate staff member. You may also leave a message with our switchboard operator at the end of the automated prompts. Questions regarding your healthcare will be directed by way of a message to the doctor's nurse. She is capable of dealing with the majority of your questions. This method of handling calls promotes continuity of care and minimizes waiting room time for the patients who have appointments to see the doctor that day. However, if your need requires that the doctor personally speak with you, every effort will be made to return your call at the end of the day when the doctor has completed seeing patients. If the call is urgent, the nurse will try to return your call as soon as possible. In addition, all calls received after 4:00 p.m., unless an emergency, may not receive a return call until the following day. Prescription Refills Prescriptions may be refilled by calling the pharmacy with the prescription number on the medication bottle. The pharmacy will phone our office for authorization. Please make your refill requests at least 2 days before you run out of medicine. When your doctor feels it is necessary, you will be asked to make an appointment with your doctor to reassess your need for the medication prior to getting a refill. Group has 4 physicians, we ask that you establish your care with one
provider who you are comfortable with. This leads to better
continuity of care. At times, due to an urgent medical problem or
when your physician is unavailable, you may see another doctor in our
group. The similarities in our approach to medical treatment and
having your medical records available makes this possible. |
